WPForms: How to Create a Custom Donation Form in WordPress
Here is a step by step guide to creating a custom donation form in WordPress.
Step 1: Install and Activate WPForms
First of all, Install and activate the WPForms plugin on your WordPress site.
Once WPForms is installed, visit the WPForms > Settings page to enter your license key. (this will connect your WordPress and WPForms account)

After that, you’ll need to log into your account at WPForms and download the plugin that you want to use it. The active plugins are different by licenses. In order to create a custom donation form in WordPress, your license should be Pro or Elite.
Step 2: Create a Donation Form
After installed and activated WPForms, click on WPForms > Add New to create a new form.
On the setup screen, you’ll see the Form Name text field. Name your form and select the Donation Form template.

A template is a pre-created form and you can customize it. The default template has Name, Email, Donation Amount, Comment, or Message fields. You can add additional fields to your donation form by dragging them from the left-hand panel to the right-hand panel.

By default, your donation form field is a Single Item field that gives users a chance to enter the amount they want to donate. But you can change it to multiple-choice or dropdown fields if you want to.
For example, drag and drop the Multiple Items form field. And add Items as another donation amount. Don’t forget to click the Required checkbox to be sure those filling out your donation form enter amounts before clicking submit.

Lastly, check the Enable conditional logic checkbox. Then, select the option to Show this field if the Donation Amount is Enter amount. Click the Save button and done!
Step 3: Set Up Payment Integration
To start set up payment integration, go to Settings > Payment. You can choose from Paypal or Stripe.

Step 4: Customize Your Notifications
If someone donates for your organization, you want to thank them. You can send a confirmation email after they donate or show a confirmation page on the site.
To customize the email and/or page notification, click Settings > Notifications or Confirmation.

Step 5: Place Donation Form on WordPress Site
This is the last part!

If you want to create a donation page, click Pages > Add New. Then click the Add Form button.
