How to Create a Signature Form with WPForms
Step 1: Go to the WPForms website. And login to your account.
Step 2: And then install and activate the Signatures Addon.

Step 3: Create a form. Start with a Blank Form template to create a WordPress contract agreement. Under the Fancy Fields section, you’ll find the Signature form field. Click the Signature form button to add it.

Step 4: Configure form settings. There are many settings you can configure on a WordPress form like your form’s name, description, submit button, etc.

Step 5: Publish your service agreement! To add the service agreement form, click on the Add Block (+) icon. Then, search for the WPForms using the search box and click on it. Select your service agreement form from the dropdown box to add it to your page. Lastly, click on the Publish button. Done!
